Fifth game of the Touhou series and the last for the PC-98 generation - released 30 December 1998 - before going into a 4-year hiatus.

For no apparent reason, demons and youkai overran Gensokyo once more, this time from a cave leading to Makai. Hakurei Reimu, Kirisame Marisa, Mima and Kazami Yuuka, each with their own agenda, decide to enter the cave in search of the root of the problem.

The game also marks the first appearance of Alice Margatroid, Stage 3 and Extra Stage Boss and later a recurring playable character for the Windows generation.
